Show moreWhile there is increasing academic analysis and policy concern regarding growing inequality and the need for more inclusive development trajectories, it is equally important to advance our understanding of the pathways to attain more inclusive development in practice. This paper serves as the introduction to a special issue examining the empirical outcomes and processes of inclusive development policies in selected countries in Africa. The paper presents a policy implementation and assessment framework as a lens that connects the diferent case studies. The framework links general inclusive development strategies in employment, social protection and governance, to the participation and representation of the various stakeholders as well as the monetary and non-monetary transaction costs in accessing and/or implementing these programmes on the ground in diferent national and sub-national contexts. Based on the fndings of the 9 case studies, the paper also advances policy directions and operational frameworks to attain more inclusive development in practice.[Résumé]. Bien qu’il existe une analyse académique croissante, et une préoccupation politique envers la montée des inégalités et la nécessite de trajectoires de développement plus inclusives, il est tout aussi important d’avancer notre compréhension des chemins qui permettent—dans la pratique—d’atteindre un développement plus inclusif. Cet article sert d’introduction à un numéro spécial examinant les résultats empiriques et les processus de développement inclusifs dans certains pays en Afrique. L’article présente un cadre d’implémentation et évaluation des politiques, tel que un prisme qui connecte les diférents études de cas. Ce cadre relie des stratégies de développement générales (dans l’emploi, la protections sociale, et la gouvernance) à la participation et la représentation de diférents partis intéressés, aussi qu’aux aux couts de transaction (monétaires et autres) dans l’accès à ces programmes et à leur implémentation sur le terrain, dans des contextes nationales et sous-nationales. Show moreBackground and objectives Treatment availability and comprehensive care have resulted in improved clinical outcomes for persons with hemophilia. Recent data on socioeconomic participation in the Netherlands are lacking. This study assessed participation in education, in the labor market, and social participation for persons with hemophilia compared with the general male population. Methods Dutch adults and children (5-75 years) of all hemophilia severities (n = 1009) participated in a questionnaire study that included sociodemographic, occupational, and educational variables. Clinical characteristics were extracted from electronic medical records. General population data were extracted from Statistics Netherlands. Social participation was assessed with the PROMIS Ability to Participate in Social Roles and Activities short form, with a minimal important difference set at 1.0. Results Data from 906 adults and children were analyzed. Participation in education of 20 to 24 year olds was 68% (general male population: 53%). Educational attainment was higher compared with Dutch males, especially for severe hemophilia. Absenteeism from school was more common than in the general population. The employment-to-population ratio and occupational disability were worse for severe hemophilia than in the general population (64.3% vs. 73.2% and 14.7% vs. 4.8%, respectively), but similar for nonsevere hemophilia. Unemployment was 5.4% (general male population: 3.4%). Absenteeism from work was less common (38% vs. 45.2%). Mean PROMIS score was similar to or higher than in the general population (54.2; SD 8.9 vs. 50; SD 10). Conclusion Socioeconomic participation of persons with nonsevere hemophilia was similar to the general male population. Some participation outcomes for persons with severe hemophilia were reduced. This study examined work difficulties and their relationship with anxiety, depression and coping style in people with MS. Methods 219 employed people with MS (median age = 43 years, 79% female) completed questionnaires on anxiety, depression, coping style, demographics and work difficulties, and underwent a neurological examination. Two regression analyses were performed with work difficulties as the dependent variable and either anxiety or depression as continuous independent variables. Coping style, age, gender, educational level, MS-related disability and disease duration were added as additional predictors, as well as interaction terms between coping style and either symptoms of depression or anxiety. Results A significant model was found (F-(10,F-205) = 13.14, p < 0.001, R-2 = 0.39) in which anxiety, emotion- and avoidance-oriented coping and MS-related disability were positively related to work difficulties. The analysis of depression resulted in a significant model (F-(10,F-205) = 14.98, p < 0.001, R-2 = 0.42) in which depression, emotion- and avoidance-oriented coping and MS-related disability were positively related to work difficulties. None of the interaction effects were significant. Conclusions Work difficulties were positively related to anxiety, depression, emotion- and avoidance-oriented coping and MS-related disability in workers with MS. Show less

Show morePurpose This study aimed to identify mental health, physical health, demographic and disease characteristics relating to work productivity in people with multiple sclerosis (MS). Methods In this cross-sectional study, 236 employed people with MS (median age = 42 years, 78.8% female) underwent neurological and neuropsychological assessments. Additionally, they completed questionnaires inquiring about work productivity (presenteeism: reduced productivity while working, and absenteeism: loss of productivity due to absence from work), mental and physical health, demographic and disease characteristics. Multiple linear and logistic regression analyses were performed with presenteeism and absenteeism as dependent variables, respectively. Results A model with mental and physical health factors significantly predicted presenteeism F(11,202) = 11.33, p < 0.001, R-2 = 0.38; a higher cognitive (p < 0.001) and physical impact (p = 0.042) of fatigue were associated with more presenteeism. A model with only mental health factors significantly predicted absenteeism; chi(2)(11)=37.72, p < 0.001, with R-2 = 0.27 (Nagelkerke) and R-2 = 0.16 (Cox and Snell). Specifically, we observed that more symptoms of depression (p = 0.041) and a higher cognitive impact of fatigue (p = 0.011) were significantly associated with more absenteeism. Conclusions In people with MS, both cognitive and physical impact of fatigue are positively related to presenteeism, while symptoms of depression and cognitive impact of fatigue are positively related to absenteeism. Show moreObjective. Long-term outcome data in adults with childhood-onset systemic lupus erythematosus (cSLE) are limited. Here, we report the effects of cSLE on education, vocation, and employment in a large cohort of adults with cSLE.Methods. Patients were seen for a single study visit comprising a structured history and physical examination. Medical records were retrieved to supplement information obtained during the study visit. Education and employment status were assessed by questionnaires. I Iealth-related quality of life (HRQOL) was measured with the 36-Item Short Form Health Survey (SF-36).Results. One hundred six patients with cSLE (93% female, 73% White), with a median disease duration of 20 years, completed the visit and questionnaires. Almost all patients stated that cSLE had influenced their education, but the level of completed education was similar to the general Dutch population. Half of the patients had adjusted their vocational choice due to the disease. Still, 44% of patients who had finished education did not have a paid job. Of the employed patients, 61% worked part time. Disease damage was equally prevalent in patients with and without paid employment. A high percentage of patients (51%) were declared work disabled, due to disease damage. Patients who did not have paid employment were often work disabled. Both had a negative effect on HRQOL.Conclusion. The effect of cSLE on academic achievements and employment is substantial, despite patients adjusting their educational and vocational choices. To optimize participation in the community, ongoing support is necessary, not only to help patients find suitable education and vocations but also to offer guidance regarding potential adjustments during their career. Show moreBackground: Huntington's disease (HD) is an inherited neurodegenerative disorder that is characterized by motor, cognitive, and psychiatric symptoms. Although 65% of HD expanded gene carriers report changes in employment as the first functional loss, little is known about the predictors leading to changes of working capacity. Given the impact on quality of life, understanding of these factors is of great clinical value.Objective: This study evaluates disease specific characteristics and their predictive value in loss of working capacity in HD.Methods: Longitudinal data was collected through the worldwide observational study (Enroll-HD), with 15,301 participants in total and 2,791 HD and healthy control participants meeting the inclusion criteria. Changes in working capacity were analyzed by means of a survival analysis. Predictive values of demographic factors and clinical characteristics were assessed for premanifest and manifest HD through Cox regressions.Results: HD expanded gene carriers, manifest and premanifest combined, had a 31% chance of experiencing changes in employment after three years, compared to 4% in healthy controls. Apathy was found to be the most crucial determinant of working capacity changes in premanifest HD, while executive and motor dysfunction play an important role in manifest HD.Conclusion: HD expanded gene carriers are more likely to lose working capacity compared to healthy controls. Disease progression, altered motor function, cognitive decline, and in an early stage of the disease apathetic symptoms are indicative of negative changes in working capacity. Clinicians should recognize that early disease related changes, especially apathy, can affect working capacity. Show less

Unfortunately, with the erosion of the manufacturing economy and the increase in the service-dominated economy, offenders who are typically on the margins of society, are pushed towards the informal economy now more than before. The current study attends to the relationship between informal work and crime by analyzing data from the Pathways to Desistance study. Results from fixed effects linear probability models show that informal work is associated with a higher probability of engaging in expressive crimes, but not instrumental crimes. Neither informal nor formal work arrangements seem to work as crime suppressants, but informal arrangements appear more criminogenic. Show less

Show moreThis thesis contains four empirical studies on the effects of social protection. In the first two studies, I use administrative microdata and employ quasi-experimental methods in which I compare a treatment group with a control group. Study 1 concludes that mandatory activation programs for young welfare recipients do not contribute to employment or to enrollment in education during an economic downturn, but are harmful to income protection. Study 2 concludes that the unemployment shock of a male partner has a large and persistent effect on household income and only 2-5% of this income loss is compensated by increases in female partner’s earnings from work. The last two studies use OECD panel data and employ international comparative research methods. Study 3 concludes that constitutional rights to social security have a positive effect on social expenditure, which is larger for expenditure targeted at the unemployed, a group that is perceived as less deserving by the public opinion compared to the elderly or disabled. Study 4 concludes that Social expenditure reduces poverty and inequality without being harmful for GDP growth. Targeted schemes are most effective in reducing poverty, while social expenditure types with a universal character are more effective in reducing inequality. Show less

Show moreEnrolment in both primary and secondary schools has risen substantially over the years, but secondary and tertiary enrolment remains low. The primary student population increased from just under 4 million students in 1970 to more than 26 million students in 2013.Similarly, at the secondary level, student numbers increased from a base of approximately 360,000 students in 1970 to 12.5 million students in 2013. Likewise, the tertiary sector recorded an appreciable increase in the numbers of institutions, variety of courses and enrolled students in the same period. Despite these increases, only about half of all primary students continue to the secondary level, and less than 15 percent eventually make it to tertiary level. The seeming lack of employment opportunities and other problems identified in the tertiary sector has contributed to a substantial increase in the number of Nigerian students undertaking tertiary education abroad, with the United Kingdom and the United States emerging as the top two destinations.Unemployment is high, particularly among youth and higher educated students. Despite Nigeria’s natural resource wealth, many Nigerians appear to have been left behind, primarily because of the shortage of employment opportunities. Unemployment is highest among youth and among those with post-secondary education. Agriculture still has the largest share of employment. Trade emerged as the second most important employment sector providing jobs for Nigerians. Though manufacturing ranks third on the list, the proportion employed in this sector has shrunk to half of what it used to be in the preceding decade. One interesting sector is Information and Communication: while providing less than 1% of all jobs, it contributes more to Nigeria’s GDP than manufacturing, due to its high value added.Nigeria is an important area of origin, destination, and transit of migrants in Africa. It is Africa’s most populous country, and a country of high migration turnover. The high immigration rate (1.2 million in 2017) is accompanied by an even higher rate of emigration of Nigerians (1.3 million in 2017), resulting in a negative net migration rate. The Nigerian Government formally adopted a National Policy on Migration and its Implementation Plan in May 2015. This occurred in the backdrop of a growing recognition of the strong links between migration and potentially immense national development benefits for the country. For instance, Central Bank of Nigeria (CBN) stated that the inflow of remittances to Nigeria increased dramatically from $2.3 billion in 2004 to $22 billion in 2018; The US, followed by the UK and Cameroon are the top three destinations of Nigerian migrants. In mainland Western Europe, the Netherlands ranks as the fifth destination behind Germany, Italy, Spain and Austria. Noteworthy is the large ‘brain drain’ of Nigeria-trained medical students and health workers as well as academics who appear to be fleeing Nigeria’s struggling tertiary education landscape and unstable labour market prospects. Thousands of them are now gainfully employed in medical jobs abroad, and they tend to prefer the US and the UK to the Netherlands. Show less

Show moreThe flower sector in Kenya accounted for over 65 per cent of new jobs created in the agricultural sector between 2010 and 2015 and remains a major employer in the country. The floriculture value chain involves a variety of players involved in activities, such as plant development and growing, providing necessary inputs, transport and handling – each of them generating a part of the direct or indirect employment. The quality of this employment is, however, less clear.In 1978, the flowers’ export volume was 3,000 tons. Since then, the sector has grown by an average of 16 per cent per year in terms of volume and 25 per cent in terms of value of production. Foreign investors and partners played a critical role in launching and expanding the floriculture industry in Kenya. Dutch companies started flower export businesses, and Dutch and Israeli advisors have been important sources of technical support. Initially, large and medium flower farms were mostly owned by foreigners, Kenyans of foreign descent or members of the Kenyan (political) elite. Kenyan smallholders were also involved in flower growing at that stage. In 2016, approximately 190 flower farms and 5,000 smallholder farmers were involved in the flower production. Today, flower growing is done predominantly by Kenyan-owned farms, while Dutch companies dominate the breeding and propagation activities. Growers provide the highest employment opportunities in the entire chain, predominantly for women, due to the labour intensive character of their operations. Breeders, propagators and other companies in the chain hire less people directly but create important indirect employment in the sector.The support of the Kenyan government in promoting the floriculture sector was mixed through the years and was not the decisive factor in the sector development. The success of the industry is, to an extent, a result of the capacity of the private sector to develop independently from the state and the capacity to quickly adapt to changing circumstances. Following the rapid development of the sector in the mid-1990s, flower companies have been criticized for poor labour standards and a number of other abusive practices. In response to this criticism, the companies have adapted a range of international and national social and environmental standards that should lead to improved labour conditions.This working paper’s primary objective is to assess to the extent to which the flower sector and the jobs created have been sustainable, inclusive and productive; and whether there are any differences between practices and quality of employment created by the international players, particularly Dutch companies in comparison to the non-Dutch counterparts. The secondary objective is to assess and understand what are the main challenges for the companies in the flower sector to create and maintain (productive) employment, as well as other pitfalls in their day-to-day operations. For this purpose, a mixed methods approach to data collection was implemented, including a sector-wide standardized questionnaire, case studies and a number of key informant interviews. This report concludes with a set of policy recommendations targeting the Kenyan government and the international community. Show less

Show moreOff-the-books employment following release from prison Prior re-entry studies used administrative data to conclude that post-release employment rates are low. These studies may however underestimate ex-prisoners’ labour market participation as especially ex-prisoners are expected to work off-the-books. A prison record might for instance create a legal ban on certain occupations, pushing them towards the informal labour market. This study combines administrative with interview data to examine the prevalence and relative importance of informal employment among former prisoners in the Netherlands (N = 738). Multiple measurement strategies show that a substantial part of these men work off-the-books. About forty percent of these workers rely on it as their primary source of income, but many combine it with a formal job or social benefits. These findings raise concern and show that many post-prison jobs currently do not come to the attention of scholars or governmental organizations. Show less

Several theories state that employment can reduce these risks but emphasize that the protective role of employment is conditional on job qualities (work intensity, job duration, etc.). Longitudinal research on the role of employment in ex-prisoners’ recidivism patterns is scarce, and most existing work used a simplistic employment measure (i.e., employed vs. unemployed), leaving the topic of job quality underexplored. This study examines the association between employment characteristics and recidivism among Dutch ex-prisoners. Using longitudinal data of the Prison Project (n = 714), we found that not just any job, but particularly stable employment and jobs with a higher occupational level could help reduce crime rates among these high-risk offenders. Many ex-prisoners face a human capital deficit that complicates the guidance to high-quality jobs. It might, however, be possible to help place ex-prisoners in stable employment. Show less

Show moreThis study examines to what extent ex-prisoners return to their pre-prison job and identifies factors that facilitate or hinder this outcome. Data from a longitudinal study of Dutch pretrial detainees were analyzed to examine whether those who were employed at the time of arrest returned to their pre-prison employer, found new employment, or remained jobless in the first half year following prison. Results show that one in three employed ex-prisoners found employment through their previous employer. The findings emphasize the relevance of recent employment ties for successful reintegration and add nuance to the assumption that employers are reluctant to hire this group of workers. Show less
