Show moreThis study seeks to determine whether white matter integrity in the brain differs between adolescents with post-traumatic stress disorder (PTSD) due to childhood sexual abuse (CSA) and matched healthy adolescents and whether there is a relationship between white matter integrity and symptom severity in the patient group. Using 3T diffusion tensor imaging, we examined fractional anisotropy (FA) in a group of adolescents with CSA-related PTSD (n = 20) and matched healthy controls (n = 20), in a region of interest consisting of the bilateral uncinate fasciculus (UF), the genu, splenium and body of the corpus callosum (CC), and the bilateral cingulum. In addition, we performed an exploratory whole brain analysis. Trauma symptomatology was measured with the Trauma Symptom Checklist for Children (TSCC) to enable correlational analyses between FA differences and trauma symptomatology. The PTSD group had significantly lower FA values in the genu, midbody and splenium of the CC in comparison with controls (p < 0.05, tfce corrected). Post hoc analyses of the eigenvalues of the DTI scan showed increased radial and mean diffusivity in the patient group. In addition, we found a significant negative correlation between scores on the anger subscale of the TSCC and FA values in the left body of the CC in patients (p < 0.05). Adolescents with CSA-related PTSD show decreased FA in the CC, with abnormalities in the integrity of the left body of the CC being related to anger symptoms. These findings suggest that early trauma exposure affects the development of the CC, which may play a role in the pathophysiology of PTSD in adolescents. Show less

Show moreDit proefschrift toont lacunes in de Nederlandse gezondheidszorg op twee deelgebieden: de cardiologie en de gastroenterologie. Hierbij is 1) zowel onder cardiologen als onder cardiologische pati_nten onderzocht in hoeverre zorg wordt geboden op het gebied van seksualiteit en seksuele disfunctie bij hart- en vaatziekten en in hoeverre er hieraan behoefte is. In het bijzonder op het gebied van erectiele disfunctie, een veel voorkomend probleem bij mannen met hart- en vaatziekten, blijkt een fors tekort te bestaan aan zorg hiervoor binnen de cardiologie. Naar aanleiding van de vergaarde data zijn richtlijnen opgesteld en aanbevelingen gedaan voor het verbeteren van de seksuele gezondheidzorg binnen de cardiologie. 2) Onder maag-darm-leverartsen en pati_nten die een colonoscopie ondergingen werd ge_valueerd in wel mate aandacht wordt besteed aan seksueel misbruik is het verleden. De impact van colonoscopisch onderzoek bij pati_nten met seksueel misbruik in de voorgeschiedenis werd aangetoond. Seksueel misbruik is zeer prevalent en heeft een bijzonder grote impact op de gezondheid en de pijnbeleving. Aanbevelingen worden gedaan voor het verbeteren van de zorg rondom colonoscopisch onderzoek voor pati_nten met seksueel misbruik in het verleden. Show less

Do urologists inquire about female sexual dysfunction and sexual abuse history? And if so, what percentage of the Dutch urologist does so? What do sexual abuse patients think about screening for sexual abuse history? Can we find predicting pelvic floor symptoms as a sign of sexual abuse history? And finally, do female urology patients with sexual abuse history more often present urological storage symptoms and/or pelvic floor complaints than patients without sexual abuse? Show less

Show moreHoe hangt kindermishandeling met gehechtheid samen, en wat zijn de risicofactoren die de kans op kindermishandeling vergroten? Het proefschrift van Eveline Euser geeft antwoord op deze vragen, en presenteert de laatste stand van zaken met betrekking tot preventie van mishandeling. Kindermishandeling en gehechtheid Alle mishandelde kinderen zijn gehecht aan hun ouders, zelfs als die ouders de daders zijn. De meta-analyse in dit proefschrift laat echter zien dat mishandelde kinderen vaker onveilig en gedesorganiseerd gehecht zijn, in vergelijking tot kinderen die opgroeien in risicogezinnen (bv. in armoede of met een drugsverslaafde ouder) maar niet mishandeld worden. Kinderen met meer dan vier risicofactoren lopen echter wel evenveel gevaar gedesorganiseerde gehechtheid te ontwikkelen. Verrassend is dus dat de opeenstapeling van sociaal-economische risico’s en kindermishandeling beide een negatieve invloed hebben op de kwaliteit van de gehechtheidsrelatie. Preventie en interventie op het gebied van kindermishandeling. In Nederland zijn momenteel geen wetenschappelijk onderbouwde interventies beschikbaar die specifiek tot doel hebben om kindermishandeling te bestrijden. Evidence-based programma's zullen daarom ontwikkeld moeten worden en getoetst in gecontroleerde experimenten. De huidige praktijk in ons land is dat preventieve opvoedingsprogramma’s die eigenlijk ontworpen zijn voor andere problematiek of in andere landen zijn beproefd (bijv. Triple P, een programma gericht op gedragsproblemen) worden ingezet bij Nederlandse gezinnen met een hoog risico op mishandeling. Uit onderzoek zal nog moeten blijken of deze preventieprogramma’s ook effectief zijn in het voorkomen van kindermishandeling. Nu worden opvoedingsprogramma’s soms overhaast breed uitgerold zonder dat er grondige experimentele evaluaties van eigen bodem voorhanden zijn. De preventie van kindermishandeling is te belangrijk om aan het toeval te worden overgelaten. Show less

Show moreIn this thesis we describe in chapter two the evaluation of female sexual function in an outpatient urologic clinic related to different urological complaints. Sexual abuse appeared to be a quite frequent problem in urological practice. In chapter three sexual abused patients are evaluated in relation to their pelvic floor complaints in order to estimate which patients are prone to have a history of sexual abuse. Chapter four reports an evaluation of a self-administered questionnaire versus a taken questionnaire administered by a pelvic floor clinician in relation to sexual abuse in patients with pelvic floor complaints. In chapter five and six we describe the influence of incontinence surgery on sexual function.. Also some novel questions are introduced to get more neuro-sexuological specific information after the incontinence operation. In chapter seven we evaluate the female sexual function and activity following cystectomy. In order to diminish the impact of a cystectomy procedure on sexual function, the effect of a partial cystectomy procedure of a 32-year-old woman with radiation cystitis is described in chapter eight.
