Show moreBackground: It is hypothesised that community-acquired pneumonia (CAP) patients with more severe disease or inflammation might benefit more from adjunctive corticosteroid treatment. Neutrophil count, lymphocyte count and neutrophil-lymphocyte ratio (NLR) have been associated with inflammation and disease severity in CAP. We investigated the interaction between these parameters and adjunctive dexamethasone effects on clinical outcomes in CAP. Methods: We conducted a post hoc analysis of the randomised placebo-controlled Santeon-CAP trial (n = 401), which showed a positive effect of adjunctive oral dexamethasone on length of stay (LOS) in CAP patients. White blood cell (WBC) count, neutrophil count, NLR (highest tertile vs. lowest two tertiles) and lymphocyte count (lowest tertile vs. highest two tertiles) were examined as potential effect modifiers of treatment with dexamethasone on LOS (primary outcome) and ICU-admission, 30-day mortality and hospital readmission. Results: WBC differential counts were available for 354 patients. The effect of dexamethasone on LOS was more pronounced in high WBC count, high neutrophil count or high NLR subgroups (difference in median LOS of 2 days versus zero days in the reference subgroups, p for interaction < 0.05). There was no effect modification for the other outcomes. Patients with low WBC and low neutrophil counts did not benefit from dexamethasone, while hospital readmission rate was higher in those treated with dexamethasone (6% vs. 11%). Conclusions: WBC count and/or neutrophil might be easily available biomarkers to guide selection of CAP patients who are more likely to benefit from adjunctive dexamethasone treatment. Future prospective trials are needed to confirm this predictive potential. Show less

Show moreDe activatie van zowel witte bloedcellen als complement systeem (betrokken bij het immuunsysteem) zijn geassocieerd met atherosclerose (slagaderverkalking). Het proefschrift bestaat uit drie delen; In deel I laten wij in 3 hoofdstukken zien dat vetten belangrijker zijn dan glucose voor de activatie van de witte bloedcellen en dus meer in staat zijn om een ontstekingsfenomeen op gang te brengen die leidt tot slagaderverkalking. Verder laten wij zien dat pati_nten met familiair verhoogd cholesterol (FH) en familiair verhoogd cholesterol en verhoogde voedingsvetten (triglyceriden) (FGH) na glucose-belasting een blijvende witte bloedcelactivatie hebben in tegenstelling tot gezonde vrijwilligers. Tenslotte werd aangetoond dat het structurele eiwit van de partikels waarin voedingsvetten vanuit de darmen worden vervoerd (apoB48) geassocieerd is met intima media dikte (een marker voor slagaderverkalking) In deel II associ_ren wij in 3 hoofstukken witte bloedcelactivatie met slagaderverkalking. Zo werd bijvoorbeeld aangetoond dat er in de kransslagaders met slagaderverkalking een hogere witte bloedcelactivatiestatus is dan in andere kransslagaders en bloedvaten zonder slagaderverkalking. Verder is de activatie van monocyten (een subgroep van witte bloedcellen) een voorspeller voor toekomstige hart- en vaatziekten. In deel III zijn ook 3 hoofdstukken beschreven met daarin de rol van mannose bindend lectine (MBL) die een van de routes is van complementactivatie (betrokken bij immuniteit; opruimen van lichaamsvreemde stoffen zoals bacteri_n). In hoofdstuk 1 werd beschreven dat mensen die lage MBL-waarden hebben, een minder effici_ntere vetstofwisseling hebben dan mensen met voldoende MBL-waarden. Echter, als we puur naar slagaderverkalking kijken, zien we dat MBL geen invloed heeft op de progressie van krasslagaderverkalking bij mensen met pre-existent krasslagaderverkalking. Show less

Show moreIn peripheral arterial occlusive disease (PAOD), the formation of an atherosclerotic lesion eventually results in a significant stenosis of a major artery thereby disrupting blood flow in peripheral arteries towards lower limb tissue. Unfortunately, there is a substantial number of patients that suffer from severe critical limb ischemia, which is associated with a poor prognosis and high rates of amputation and mortality. Despite state-of-the-art revascularization treatment options and optimal control of co-morbidities these 'no option' patients remain at high risk for limb amputation. It is these patients that require new therapeutic applications to augment neovascularization and prevent them from limb amputation. Expansion of our current knowledge is required to gain insight in cellular and molecular mechanisms that are involved in neovascularization, to optimize revascularization therapies for patients with critical limb ischemia. This thesis provides a role for multiple leukocytes in blood flow recovery, in particular Natural Killer cells and CD4+ T lymphocytes, and shows that genetic differences in the Natural Killer gene complex induce differences in vascular remodeling. It was also shown how growth factor expression, signaling and regulation of gene expression modulate revascularization. Show less
