Show moreThe main aim of this dissertation was to provide insight into possible subtle forms of bias in youth socialization by various socializing agents in the Netherlands, and exploring new ways to document bias. Additionally, it addresses the importance of using sound statistical methods for calculating effect sizes. Social bias and unequal treatment of people can lead to structural societal inequalities. Findings in research on the sustained existence of bias have been inconsistent. These inconsistencies may obscure the importance of continuous effort to diminish inequalities. By studying subtle messages, this dissertation shows that bias is present in various contexts. Firstly, parents who talked about kissing and dating to their adolescent child subtly endorsed a sexual double standard, for instance by describing girls as sluts and boys as players. Secondly, in written evaluations by teachers on report cards of primary school pupils, positive bias towards White Dutch girls was found. Lastly, prolonged media attention to a child sexual abuse case was found to negatively affect the attitudes of parents towards male babysitters.As bias and discrimination change over time and context, continuous study is necessary. This dissertation demonstrates that using relevant and authentic data is an important tool in unveiling subtle bias. Show less

Show moreBackground Socioeconomic status and ethnicity are not explicitly incorporated as risk factors in the four SCORE2 cardiovascular disease (CVD) risk models developed for country-wide implementation across Europe (low, moderate, high and very-high model). The aim of this study was to evaluate the performance of the four SCORE2 CVD risk prediction models in an ethnic and socioeconomic diverse population in the Netherlands.Methods The SCORE2 CVD risk models were externally validated in socioeconomic and ethnic (by country of origin) subgroups, from a population-based cohort in the Netherlands, with GP, hospital and registry data. In total 155,000 individuals, between 40 and 70 years old in the study period from 2007 to 2020 and without previous CVD or diabetes were included. Variables (age, sex, smoking status, blood pressure, cholesterol) and outcome first CVD event (stroke, myocardial infarction, CVD death) were consistent with SCORE2. Findings 6966 CVD events were observed, versus 5495 events predicted by the CVD low-risk model (intended for use in the Netherlands). Relative underprediction was similar in men and women (observed/predicted (OE-ratio), 1.3 and 1.2 in men and women, respectively). Underprediction was larger in low socioeconomic subgroups of the overall study population (OE-ratio 1.5 and 1.6 in men and women, respectively), and comparable in Dutch and the combined "other ethnicities" low socioeconomic subgroups. Underprediction in the Surinamese subgroup was largest (OE-ratio 1.9, in men and women), particularly in the low socioeconomic Surinamese subgroups (OE-ratio 2.5 and 2.1 in men and women). In the subgroups with underprediction in the low-risk model, the intermediate or high-risk SCORE2 models showed improved OE-ratios. Discrimination showed moderate performance in all subgroups and the four SCORE2 models, with C-statistics between 0.65 and 0.72, similar to the SCORE2 model development study.Interpretation The SCORE 2 CVD risk model for low-risk countries (as the Netherlands are) was found to underpredict CVD risk, particularly in low socioeconomic and Surinamese ethnic subgroups. Including socioeconomic status and ethnicity as predictors in CVD risk models and implementing CVD risk adjustment within countries is desirable for adequate CVD risk prediction and counselling. Show moreBackground: Sex-specific thresholds of computed tomography (CT)-derived aortic valve calcification (AVC) or AVC density (AVCd) to identify severe aortic stenosis (AS) have been established in populations that consisted mainly of Caucasians with a tricuspid aortic valve. The objective of this study was to evaluate the accuracy (i.e., sensitivity and specificity) of previously established thresholds to identify severe AS in patients with bicuspid aortic valve (BAV) and according to ethnicity: Caucasian vs. Asian. Methods: We built a multicenter registry of echocardiographic and CT data collected in BAV patients with at least mild AS and preserved left ventricular ejection fraction from 7 different centers. Anatomic severity of AS obtained by CT-derived AVC and AVCd was compared to hemodynamic severity of AS obtained by echocardiography. Results: Among 485 BAV patients (60% men, 73% Asians), the best thresholds of AVC and AVCd to identify severe AS in BAV patients were 2315 arbitrary units (AU) (sensitivity [Se]/specificity [Spe] = 82/78%) in men, 1103 AU (Se/Spe = 80/82%) in women, and 561 AU/cm2 (Se/Spe = 86/91%) in men, and 301 AU/cm2 (Se/Spe = 83/ 82%) in women, respectively. According to ethnicity, thresholds for severe AS in Caucasian patients were, respectively, in men and women: 2208 AU (Se/Spe = 83/83%) and 1230 AU (Se/Spe = 87/82%) for AVC and 474 AU/cm2 (Se/Spe = 88/83%) and 358 AU/cm2 (Se/Spe = 80/82%) for AVCd. In Asian patients, they were 2582 AU (Se/Spe = 76/78%) and 924 AU (Se/Spe = 84/80%) for AVC and 640 AU/cm2 (Se/Spe = 82/89%) and 255 AU/cm2 (Se/Spe = 86/80%) for AVCd. Conclusions: The optimal thresholds to identify hemodynamically severe AS in BAV patients are similar in Caucasians but appear to be higher in Asian men, compared with thresholds previously reported in tricuspid aortic valve patients. Nonetheless, the thresholds currently proposed in the guidelines have good accuracy and can be applied in BAV patients to confirm AS severity. Show less

Previous studies on the relationship between intergroup contact and conflict onset have produced mixed findings. Some scholars have argued that this might be due to the way interethnic contact is measured. Building on insights of social psychology, this study examines the influence of interethnic marriage as the ultimate form of positive intergroup contact. In doing so, we combine information of the Demographic and Health Surveys with event data on ethnic conflict. Our analysis shows that areas with higher levels of exogamy significantly experience less ethnic conflict. Show less

Show moreBackground Ethnic disparities have been reported in cardiovascular disease. However, ethnic disparities in takotsubo syndrome (TTS) remain elusive. This study assessed differences in clinical characteristics between Japanese and European TTS patients and determined the impact of ethnicity on in-hospital outcomes.Methods TTS patients in Japan were enrolled from 10 hospitals and TTS patients in Europe were enrolled from 32 hospitals participating in the International Takotsubo Registry. Clinical characteristics and in-hospital outcomes were compared between Japanese and European patients.Results A total of 503 Japanese and 1670 European patients were included. Japanese patients were older (72.6 +/- 11.4 years vs. 68.0 +/- 12.0 years; p < 0.001) and more likely to be male (18.5 vs. 8.4%; p< 0.001) than European TTS patients. Physical triggering factors were more common (45.5 vs. 32.0%; p < 0.001), and emotional triggers less common (17.5 vs. 31.5%; p < 0.001), in Japanese patients than in European patients. Japanese patients were more likely to experience cardiogenic shock during the acute phase (15.5 vs. 9.0%; p < 0.001) and had a higher in-hospital mortality (8.2 vs. 3.2%; p< 0.001). However, ethnicity itself did not appear to have an impact on in-hospital mortality. Machine learning approach revealed that the presence of physical stressors was the most important prognostic factor in both Japanese and European TTS patients.Conclusion Differences in clinical characteristics and in-hospital outcomes between Japanese and European TTS patients exist. Ethnicity does not impact the outcome in TTS patients. The worse in-hospital outcome in Japanese patients, is mainly driven by the higher prevalence of physical triggers. Show less

Show moreA substantial body of research has found that social desirability motivates respondents to overstate support for immigration when asked directly, but when provided an unobtrusive means of expressing preferences, support declines. In this paper, we ask whether South Koreans follow this pattern, especially with regards to co-ethnic migrants from North Korea and China. We use list experiments to determine whether observed levels of support for general immigration and co-ethnic migration are biased by social desirability. We find that generally respondents overstate their support for co-ethnics from North Korea by a significant amount when asked directly, but not for the other groups, although college-educated respondents overstate their support for general immigration. Social desirability bias with respect to co-ethnics from North Korea is particularly evident in older respondents and males. These findings challenge the notion that native-born citizens prefer co-ethnic immigrants. Show less

Show moreThe aim of this dissertation is to explore what adaptations are needed to improve the fit of a parenting intervention for use in multi-ethnic disadvantaged neighborhoods. A bottom-up approach is used to assess parents’ perspectives regarding their parenting experiences, and to identify their needs and stressors. Data is collected among parents with an ethnic minority and low socioeconomic status background. Each chapter is a part of a stepwise approach that led to the selection of an intervention, and its adaptation for and evaluation in a multi-ethnic disadvantaged neighbourhood. It was found that a focus on coping strategies to handle stress and unhelpful emotions is not a standard theme in parenting interventions, while this would be beneficial for ethnic minority and low SES parents. The intervention module ‘Coping with stress and unhelpful emotions’ was developed to be incorporated into an existing parenting intervention to assist parents in handling their emotions and decreasing the impact of stress, and simultaneously regulating their parenting behavior. An evaluation study showed the relevance of this in-built module for parents living in multi-ethnic disadvantaged neighborhoods and also yielded recommendations to further improve the module. Show moreThis study analyses the everyday practices and the multiplicious identification processes in Amazigh mineral and fossil artisan communities, focusing on the Ayt Khebbash group in Rissani and Tafraoute of southeastern Morocco. I address the following questions: how do the Ayt Khebbash artisans identify themselves and others through everyday work and participation in different communities? What is their relation to the space that surrounds them? How do they deal with the discourse power of the state and the penetrating influences of capitalism? I argue that in the context of the Tafilalet, the collective Amazigh ethnic identity promoted by the activists holds little importance compared to a tribal sense of belonging based on agnatic ties. Also, the local Ayt Khebbash artisans see themselves contextually through social interactions in everyday work and in relation to external power structures. This dissertation consists of an introduction, four main chapters, and a conclusion. The division of the main content of this study into four chapters is in accordance with the geography of the region: Tafilalet (Ar-Rachidia Province), the village Tafraoute, the town Rissani, and reḥla (‘open space’). I examine the development of fossil ex¬traction and sculpting work in Tafraoute and Rissani in the context of environmental factors and the local history of French colonial rule, so as to situate the practice of Ayt Khebbash men in the historical process of herding, agriculture and mining work. Then I analyse the apprenticeship of fossil artisans as a process of participation in social practice, by using the notion of ‘legitimate peripheral participation’ within the ‘communities of practice’ developed by Lave and Wenger (Lave and Wenger 1991; Wenger 1998). This is to demonstrate that work is a social practice wherein acting agents acquire new knowledge, techniques, and lifestyles, in the multiple processes of confrontations and contradictions. Furthermore, through an analysis of the Ayt Kheb¬bash artisans’ work experiences in Nador and barite mining, I argue that their identifi¬cation processes also involve ‘marginality’, ‘dis-identification’, and ‘non-participation’ (Hodges 1998). Contrary to Lave and Wenger’s model (1991; 1998), ‘participation’ was an experience in constant conflict with their historically situated self within the new socio-economic frameworks, which in the case of the Ayt Khebbash artisans induced sympathy for their own tribal identities, rather than assimilation to the structural conditions of the capitalist labour market. I conclude that the Ayt Khebbash artisans counteract the effect of globalisation in their own terms with their own initiatives, by constantly imagining, reinventing and reconstructing their spatial and tribal senses of belonging. It includes analyses and discussion of minority groups’ self-perception within broader national contexts, self-esteem as a result of religious group membership, and the dichotomy between religious in-group identity and active belief. If we are able to distinguish ‘belief’ from ‘belonging’ relative to institutional religions, we might better accommodate the needs and values of these groups. This book focuses on a Canadian group of secular Jews, combining quantitative and qualitative methods to illuminate how religious identity, connection and membership affect daily modern life. Show morePeople of South Asian origin have an increased risk of developing type 2 diabetes (T2D) and cardiovascular disease (CVD) compared to people of Western European descent. Not only is the prevalence of these diseases higher in South Asians, they also occur at a younger age and lower BMI, and have a more severe course. The high prevalence of T2D and CVD in South Asians, who comprise one fifth of the total world__s population, poses a major health and socioeconomic burden worldwide. The underlying cause of this excess risk is, however, still poorly understood. The studies described in this thesis were performed to gain more insight in the pathogenesis of T2D and CVD in South Asians and to provide new leads for preventive strategies and treatment options. For this purpose sophisticated techniques were used such as hyperinsulinemic-euglycemic clamp with stable isotopes, indirect calorimetry, skeletal muscle biopsies, MRI and spectroscopy, and brown fat quantification using PET-CT-imaging, combined with short-term dietary interventions, in healthy lean young adult men and overweight adult men. These studies have led to a number of promising areas for further research. It seems that not one, but multiple metabolic mechanisms have been affected, most likely due to gene-environment interactions. Show less
