Het eerste deel van het proefschrift geeft een overzicht van de huidige inzichten over het ontstaan van galwegstricturen. Het blijkt dat de hoogte van het serum piek alanine-aminotransferase na transplantatie (mate van ischemie-reperfusieschade) een goede voorspeller is voor het ontstaan van galwegstricturen na transplantatie van een lever afkomstig van een hartdode donor. Ook werd de relatie van galwegstricturen met de vorming van donor-specifieke antilichamen onderzocht. Deze relatie werd niet gevonden, maar de aanwezigheid van antilichamen bleek wel geassocieerd met een verminderde graft survival. Het tweede deel van het proefschrift is gericht op niet-invasieve diagnostiek. Zo wordt een gevalideerd scoringssysteem beschreven waarbij het mogelijk is met een MRCP betrouwbaar galwegstricturen aan te tonen. Verder bleek dat patiënten met galwegstricturen en cholestase een verhoogde elasticiteitsmeting hebben ten opzichte van patiënten met galwegstricturen maar zonder cholestase, of patiënten zonder galwegstricturen. In het derde deel wordt beschreven hoe de behandeling van galwegstricturen verloopt. Galwegstricturen bleken in 60.5% van de gevallen succesvol behandeld te kunnen worden. Multiple en intrahepatische galwegstricturen zijn meer therapieresistent hebben frequent een retransplantatie nodig. Tenslotte werd een studie uitgevoerd die onderzoekt hoe de kwaliteit van leven is van patiënten met galwegstricturen. Show less

Show moreThe current thesis describes short and long term results of orthotopic liver transplantation (OLT) performed with livers from donation after brain death (DBD) and livers from donation after cardiac death (DCD) with an emphasis on biliary complications, especially nonanastomotic biliary strictures (NAS). In a 20-year single center study we have shown that long term survival has improved significantly with current 1-, 5- and 10-years survival rates of 91%, 83% en 77%, respectively. However, DCD-OLT leads to more development of NAS probably due to more ischemia/reperfusion damage as indicated by higher peaks of alanine aminotransferas (ALT) after OLT. We have shown that peak ALT level of >1300 IU/L was associated with a higher incidence of NAS and can thus be used as a clinical tool. Furthermore, the role of genetic polymorphisms of matrix metalloproteinases (MMPs), as proteolytic enzymes involved in tissue remod eling, was investigated in the relation to the development of biliary stricture formation before, i.e. in primary sclerosing cholangitis (PSC), and after OLT. We have found that MMP-2 C/T polymorphism was related to more severe progression of PSC resulting in earlier OLT or death. Show moreChimerism after orthotopic liver transplantation (OLT) is the main focus of the studies described in this thesis. The first study showed that chimerism of different cell lineages within the liver graft does occur after OLT. Subsequently, in allogeneic blood stem cell recipients, chimerism was demonstrated in liver tissue, providing evidence that circulating progenitor cells can indeed differentiate into parenchymal liver cells.The secondary focus of this thesis is on matrix metalloproteinases (MMPs). These proteolytic enzymes are involved in a wide variety of physiological and disease-related matrix remodeling processes. MMP-2 and MMP-9 gene promotor polymorphisms were assessed of OLT donors and recipients in relation to ischemia/reperfusion injury, acute rejection and non-anastomotic biliary strictures after transplantation. We performed a side study, in which we evaluated the value of serum liver chemistry profile and abdominal ultrasound for detecting clinically relevant biliary strictures using time-dependent multivariate regression analysis. The two main themes, chimerism and MMPs, merge in the final chapter of this thesis. In liver transplant recipients with donor/recipient mismatches for MMP gene polymorphisms chimerism was studied, in both liver biopsies as in peripheral blood after OLT. Show less
