Show moreChemicals emitted to the environment affect ecosystem health from local to global scale, and reducing chemical impacts has become an important element of European and global sustainability efforts. The present work ad-vances ecotoxicity characterization of chemicals in life cycle impact assessment by proposing recommendations resulting from international expert workshops and work conducted under the umbrella of the UNEP-SETAC Life Cycle Initiative in the GLAM project (Global guidance on environmental life cycle impact assessment indicators). We include specific recommendations for broadening the assessment scope through proposing to introduce additional environmental compartments beyond freshwater and related ecotoxicity indicators, as well as for adapting the ecotoxicity effect modelling approach to better reflect environmentally relevant exposure levels and including to a larger extent chronic test data. As result, we (1) propose a consistent mathematical framework for calculating freshwater ecotoxicity characterization factors and their underlying fate, exposure and effect pa-rameters; (2) implement the framework into the USEtox scientific consensus model; (3) calculate characteriza-tion factors for chemicals reported in an inventory of a life cycle assessment case study on rice production and consumption; and (4) investigate the influence of effect data selection criteria on resulting indicator scores. Our results highlight the need for careful interpretation of life cycle assessment impact scores in light of robustness of underlying species sensitivity distributions. Next steps are to apply the recommended characterization frame-work in additional case studies, and to adapt it to soil, sediment and the marine environment. Our framework is applicable for evaluating chemicals in life cycle assessment, chemical and environmental footprinting, chemical substitution, risk screening, chemical prioritization, and comparison with environmental sustainability targets. Show moreBackground and Purpose:High-serum glucose on admission is a predictor of poor outcome after stroke. We assessed the association between glucose concentrations and clinical outcomes in patients who underwent endovascular treatment.Methods:From the MR CLEAN Registry, we selected consecutive adult patients with a large vessel occlusion of the anterior circulation who underwent endovascular treatment and for whom admission glucose levels were available. We assessed the association between admission glucose and the modified Rankin Scale score at 90 days, symptomatic intracranial hemorrhage and successful reperfusion rates. Hyperglycemia was defined as admission glucose >= 7.8 mmol/L. We evaluated the association between glucose and modified Rankin Scale using multivariable ordinal logistic regression and assessed whether successful reperfusion (extended Thrombolysis in Cerebral Infarction 2b-3) modified this association.Results:Of 3637 patients in the MR CLEAN Registry, 2908 were included. Median admission glucose concentration was 6.8 mmol/L (interquartile range, 5.9-8.1) and 882 patients (30%) had hyperglycemia. Hyperglycemia on admission was associated with a shift toward worse functional outcome (median modified Rankin Scale score 4 versus 3; adjusted common odds ratio, 1.69 [95% CI, 1.44-1.99]), increased mortality (40% versus 23%; adjusted odds ratio, 1.95 [95% CI, 1.60-2.38]), and an increased risk of symptomatic intracranial hemorrhage (9% versus 5%; adjusted odds ratio, 1.94 [95% CI, 1.41-2.66]) compared with nonhyperglycemic patients. The association between admission glucose levels and poor outcome (modified Rankin Scale score 3-6) was J-shaped. Hyperglycemia was not associated with the rate of successful reperfusion nor did successful reperfusion modify the association between glucose and functional outcome.Conclusions:Increased admission glucose is associated with poor functional outcome and an increased risk of symptomatic intracranial hemorrhage after endovascular treatment. Show moreThe number of anthropogenic chemicals, manufactured, by-products, metabolites and abiotically formed transformation products, counts to hundreds of thousands, at present. Thus, humans and wildlife are exposed to complex mixtures, never one chemical at a time and rarely with only one dominating effect. Hence there is an urgent need to develop strategies on how exposure to multiple hazardous chemicals and the combination of their effects can be assessed. A workshop, "Advancing the Assessment of Chemical Mixtures and their Risks for Human Health and the Environment" was organized in May 2018 together with Joint Research Center in Ispra, EU-funded research projects and Commission Services and relevant EU agencies. This forum for researchers and policy-makers was created to discuss and identify gaps in risk assessment and governance of chemical mixtures as well as to discuss state of the art science and future research needs. Based on the presentations and discussions at this workshop we want to bring forward the following Key Messages:• We are at a turning point: multiple exposures and their combined effects require better management to protect public health and the environment from hazardous chemical mixtures.• Regulatory initiatives should be launched to investigate the opportunities for all relevant regulatory frameworks to include prospective mixture risk assessment and consider combined exposures to (real-life) chemical mixtures to humans and wildlife, across sectors.• Precautionary approaches and intermediate measures (e.g. Mixture Assessment Factor) can already be applied, although, definitive mixture risk assessments cannot be routinely conducted due to significant knowledge and data gaps.• A European strategy needs to be set, through stakeholder engagement, for the governance of combined exposure to multiple chemicals and mixtures. The strategy would include research aimed at scientific advancement in mechanistic understanding and modelling techniques, as well as research to address regulatory and policy needs. Without such a clear strategy, specific objectives and common priorities, research, and policies to address mixtures will likely remain scattered and insufficient. Show less

Show moreEcosystem quality is an important area of protection in life cycle impact assessment (LCIA). Chemical pollution has adverse impacts on ecosystems at the global scale. To improve methods for assessing ecosystem impacts, the Life Cycle Initiative hosted at the United Nations Environment Programme established a task force to evaluate the state-of-the-science in modelling chemical exposure of organisms and resulting ecotoxicological effects for use in LCIA. Outcome of the task force work will be global guidance and harmonization by recommending changes to the existing practice in exposure and effect modelling in ecotoxicity characterization. These changes reflect the current science and ensure stability of recommended practice. Recommendations must work within the needs of LCIA in terms of (a) operating on information from any inventory reporting chemical emissions with limited spatiotemporal information, (b) applying best estimates rather than conservative assumptions to ensure unbiased comparison with results for other impact categories, and (c) yielding results that are additive across substances and life cycle stages and allow a quantitative expression of damage to the exposed ecosystem. Here, we report the current framework as well as discuss research questions identified in a roadmap. Primary research questions relate to the approach for ecotoxicological effect assessment, the need to clarify the method's scope and interpretation of its results, the need to consider additional environmental compartments and impact pathways, and the relevance of effect metrics other than the currently applied geometric mean of toxicity effect data across species. Because they often dominate ecotoxicity results in LCIA, metals pose a specific focus, which includes consideration of their possible essentiality and changes in environmental bioavailability. We conclude with a summary of key questions along with preliminary recommendations to address them as well as open questions that require additional research efforts. Show moreThe European Union's REACH regulation has introduced Socio-Economic Analyses as a new decision support tool in the domain of chemical policy-making. This paper presents a pragmatic method to review the replacement of chemicals in processes or products in terms of the environmental impact. The aim of this work is to develop a broadly accepted method for environmental impact assessment as part of Socio-Economic Analyses. The method consists of a stepwise and tiered approach for Environmental Impact Assessment whereby the expected impact of the replacement of chemicals is assessed via Risk Characterization Ratios and toxic pressure quantification (expressed as a Potentially Affection Fraction ofspecies), and e if relevant e via a Persistency, Bioaccumulation potential and Toxicity score. The working of the method is demonstrated by the replacement of chemical substances in detergents, gutters and Expanded Polystyrene. Of the three replacements, for gutters, the reduction of the toxic pressure on the aquatic compartment was highest. Based on 50% effect concentrations the Potentially Affection Fraction of species due to the use of zinc gutters was relatively high (15%), while it was 0.6% after replacement by PVC gutters. This indicates that PVC gutters have a lower direct impact on aquatic biodiversity than zinc gutters. This paper demonstrates that even with limited data the proposed method can be used to move from risk indicators to impact indicators. The tiered approach allows finding the most appropriate level of analysis in a cost and resource efficient way. The method allows comparison of results of different scenarios and as such allows selecting the most preferable alternative from an environmental perspective. This is useful in the context of socio-economic analysis and as such, this method is available as a decision-support tool under REACH and other chemical policy frames such as the United Nations Environment Program. Show moreSustainability assessments can play an important role in decision making. This role starts with selecting appropriate methods for a given situation. We observed that scientists, consultants, and decision-makers often do not systematically perform a problem analyses that guides the choice of the method, partly related to a lack of systematic, though sufficiently versatile approaches to do so. Therefore, we developed and propose a new step towards method selection on the basis of question articulation: the Sustainability Assessment Identification Key. The identification key was designed to lead its user through all important choices needed for comprehensive question articulation. Subsequently, methods that fit the resulting specific questions are suggested by the key. The key consists of five domains, of which three determine method selection and two the design or use of the method. Each domain consists of four or more criteria that need specification. For example in the domain "system boundaries", amongst others, the spatial and temporal scales are specified. The key was tested (retrospectively) on a set of thirty case studies. Using the key appeared to contribute to improved: (i) transparency in the link between the question and method selection; (ii) consistency between questions asked and answers provided; and (iii) internal consistency in methodological design. There is latitude to develop the current initial key further, not only for selecting methods pertinent to a problem definition, but also as a principle for associated opportunities such as stakeholder identification.
