Theme 1: The "coagulome" as a critical driver of cardiovascular disease. Blood coagulation proteins also play divergent roles in biology and pathophysiology, related to specific organs, including brain, heart, bone marrow, and kidney. Four investigators shared their views on these organ- specific topics. Theme 2: Novel mechanisms of thrombosis. Mechanisms linking factor XII to fibrin, including their structural and physical properties, contribute to thrombosis, which is also affected by variation in microbiome status. Virus infection-associated coagulopathies perturb the hemostatic balance resulting in thrombosis and/ or bleeding. Theme 3: How to limit bleeding risks: insights from translational studies. This theme included state-of- the- art methodology for exploring the contribution of genetic determinants of a bleeding diathesis; determination of polymorphisms in genes that control the rate of metabolism by the liver of P2Y12 inhibitors, to improve safety of antithrombotic therapy. Novel reversal agents for direct oral anticoagulants are discussed. Theme 4: Hemostasis in extracorporeal systems: the value and limitations of ex vivo models. Perfusion flow chamber and nanotechnology developments are developed for studying bleeding and thrombosis tendencies. Vascularized organoids are utilized for disease modeling and drug development studies. Strategies for tackling extracorporeal membrane oxygenation-associated coagulopathy are discussed. Theme 5: Clinical dilemmas in thrombosis and antithrombotic management. Plenary presentations addressed controversial areas, i. e., thrombophilia testing, thrombosis risk assessment in hemophilia, novel antiplatelet strategies, and clinically tested factor XI(a) inhibitors, both possibly with reduced bleeding risk. Show moreBackground: Thrombosis is a frequent and severe complication in patients with coronavirus disease 2019 (COVID-19) admitted to the intensive care unit (ICU). Lupus anticoagulant (LA) is a strong acquired risk factor for thrombosis in various diseases and is frequently observed in patients with COVID-19. Whether LA is associated with thrombosis in patients with severe COVID-19 is currently unclear. Objective: To investigate if LA is associated with thrombosis in critically ill patients with COVID-19. Patients/Methods: The presence of LA and other antiphospholipid antibodies was assessed in patients with COVID-19 admitted to the ICU. LA was determined with dilute Russell's viper venom time (dRVVT) and LA-sensitive activated partial thromboplastin time (aPTT) reagents. Results: Of 169 patients with COVID-19, 116 (69%) tested positive for at least one antiphospholipid antibody upon admission to the ICU. Forty (24%) patients tested positive for LA; of whom 29 (17%) tested positive with a dRVVT, 19 (11%) tested positive with an LA-sensitive aPTT, and 8 (5%) tested positive on both tests. Fifty-eight (34%) patients developed thrombosis after ICU admission. The odds ratio (OR) for thrombosis in patients with LA based on a dRVVT was 2.5 (95% confidence interval [CI], 1.1-5.7), which increased to 4.5 (95% CI, 1.4-14.3) in patients at or below the median age in this study (64 years). LA positivity based on a dRVVT or LA-sensitive aPTT was only associated with thrombosis in patients aged less than 65 years (OR, 3.8; 95% CI, 1.3-11.4) and disappeared after adjustment for C-reactive protein. Conclusion: Lupus anticoagulant on admission is strongly associated with thrombosis in critically ill patients with COVID-19, especially in patients aged less than 65 years. Show less

Show moreThis year's Congress of the International Society of Thrombosis and Haemostasis (ISTH) was hosted virtually from Philadelphia July 17-21, 2021. The conference, now held annually, highlighted cutting-edge advances in basic, population and clinical sciences of relevance to the Society. Despite being held virtually, the 2021 congress was of the same scope and quality as an annual meeting held in person. An added feature of the program is that talks streamed at the designated times will then be available online for asynchronous viewing. The program included 77 State of the Art (SOA) talks, thematically grouped in 28 sessions, given by internationally recognized leaders in the field. The SOA speakers were invited to prepare brief illustrated reviews of their talks that were peer reviewed and are included in this article. The topics, across the main scientific themes of the congress, include Arterial Thromboembolism, Coagulation and Natural Anticoagulants, COVID-19 and Coagulation, Diagnostics and Omics, Fibrinogen, Fibrinolysis and Proteolysis, Hemophilia and Rare Bleeding Disorders, Hemostasis in Cancer, Inflammation and Immunity, Pediatrics, Platelet Disorders, von Willebrand Disease and Thrombotic Angiopathies, Platelets and Megakaryocytes, Vascular Biology, Venous Thromboembolism and Women's Health. Show moreThe incidence of venous thrombosis, mostly pulmonary embolism (PE), ranging from local immunothrombosis to central emboli, but also deep vein thrombosis (DVT) in people with coronavirus disease 2019 (COVID-19) is reported to be remarkably high. The relevance of better understanding, predicting, treating, and preventing COVID-19-associated venous thrombosis meets broad support, as can be concluded from the high number of research, review, and guideline papers that have been published on this topic. The Dutch COVID & Thrombosis Coalition (DCTC) is a multidisciplinary team involving a large number of Dutch experts in the broad area of venous thrombosis and hemostasis research, combined with experts on virology, critically ill patients, pulmonary diseases, and community medicine, across all university hospitals and many community hospitals in the Netherlands. Within the consortium, clinical data of at least 5000 admitted COVID-19-infected individuals are available, including substantial collections of biobanked materials in an estimated 3000 people. In addition to considerable experience in preclinical and clinical thrombosis research, the consortium embeds virology-hemostasis research models within unique biosafety facilities to address fundamental questions on the interaction of virus with epithelial and vascular cells, in relation to the coagulation and inflammatory system. The DCTC has initiated a comprehensive research program to answer many of the current questions on the pathophysiology and best anticoagulant treatment of COVID-19-associated thrombotic complications. The research program was funded by grants of the Netherlands Thrombosis Foundation and the Netherlands Organization for Health Research and Development. Deze en andere recente uitspraken roepen daarmee de vraag op welke rol tekstuele argumenten precies spelen bij de uitleg van de belastingwet. In deze bijdrage demonstreert de auteur aan de hand van de procedures waarin op 23 mei 2014 uitspraak is gedaan dat het zinvoller is om de taalkundige (of grammaticale) "methode" van rechtsvinding te percipiëren en te gebruiken als een categorie argumenten om een reeds gekozen standpunt te onderbouwen dan als wetenschappelijke methode om het recht vast te stellen en zo een standpunt te bepalen. Hij gaat daarbij in op zowel de positie van de procespartijen die hun belang verdedigen, als die van de rechter die het geschil moet beslechten. Show less

Show moreIn 1949 heeft de Hoge Raad de leer van het stamkapitaal afgewezen. Die leer gaat ervan uit dat er geen wanverhouding tussen eigen vermogen en vreemd vermogen mag bestaan. De wetgever heeft die afwijzing altijd onderschreven totdat naar aanleiding van het Bosal-arrest de onderkapitalisatiemaatregel van art. 10d Wet VPB 1969 (oud) is ingevoerd. Hieruit kan worden afgeleid dat een specifieke wettelijke bepaling noodzakelijk is om tot herkwalificatie van een kapitalisatie van een onderneming te komen en de leer van het stamkapitaal niet uit het stelsel volgt. In het recent bijgewerkte verrekenprijzenbesluit neemt de Staatssecretaris van Financiën niettemin in navolging van de OESO het standpunt in dat leningen waarvoor geen at arm’s length-voorwaarden kunnen worden vastgesteld kunnen worden genegeerd of geherkwalificeerd met een beroep op het in art. 8b Wet VPB 1969 gecodificeerde transfer pricing-gedachtegoed. In deze bijdrage onderzoeken de auteurs of deze stelling door de jurisprudentie van de Hoge Raad wordt gesteund. De auteurs beschrijven de benadering van de OESO en de visie in het nieuwe verrekenprijzenbesluit. Daarna toetsen zij die visie aan de huidige stand van de jurisprudentie over de toepassing van het at arm’s length-beginsel in verdragssituaties, de onzakelijke lening en de (on)mogelijkheid om civiel eigen vermogen als fiscaal vreemd vermogen aan te merken. Show moreConsidering that early modern scholars often referred to the Middle Ages as an uncouth period of darkness and ignorance, it is surprising that humanist historians by no means neglected the era. The central hypothesis of this book is that the ways in which historians such as Reynier Snoy, Adrianus Barlandus, Petrus Divaeus, and Janus Dousa Sr described the medieval past can be explained by the political context from which their writings originated and in which they were often directly involved. This context was marked by upheavals caused by factors such as the Habsburg centralization policy, the Reformation, and the Dutch Revolt. This book brings forward key characteristics of early modern medievalism, showing how concepts of the medieval were used as rhetorical tools, how medieval forms and ideals were appropriated, and how the classical heritage was involved in the representation of the medieval. This analysis is informed by an approach to historical writing that differs from what is common in the study of sixteenth-century historiography. Historiography is regarded not as a means to uncover the historical truth, but as narrative rhetoric. It deploys narrative techniques and intertextual allusions and plays with genre expectations in order to convey its message. Show less
