Show moreA group of youth with severe and enduring mental health problems (SEMHP) falls between the cracks of the child-and-adolescent psychiatry (CAP) system. An insufficient understanding of these youth's mental health problems results in a failure to accurately identify and provide support to these youth. To gain a deeper understanding, the aim of this study is to explore characteristics of youth with SEMHP in clinical practice based on the experiences of youth and clinicians in CAP. This qualitative study consisted of 20 semi-structured interviews with 10 youth with lived experience and 10 specialized clinicians in CAP. Both a thematic and content analysis was conducted to identify, assess, and report themes associated with youth with SEMHP. Themes were individual characteristics such as trauma, masking, self-destructive behavior, interpersonal distrust as well as environmental and systematic characteristics including parental stressors, social isolation and societal stressors, which go beyond the existing classifications. These characteristics profoundly impact youth's daily functioning across various life domains, creating an interactive process, ultimately leading to elusive mental health problems and overwhelming feelings of hopelessness. The authors recommend proper assessment of characteristics in all life domains affected and their perpetuating effect on SEMHP during diagnostics in CAP. Engaging in a dialogue with youth themselves is crucial due to the nature of youth's characteristics, which frequently transcend traditional classifications and may not be immediately discernible. It also requires an integrated care approach, entailing collaborations between educational institutions and mental healthcare providers, and attention to potential indicators of deficits in the healthcare system and society. Show moreThe immune checkpoint NKG2A/CD94 is a promising target for cancer immunotherapy, and its ligand major histocompatibility complex E (MHC-E) is frequently upregulated in cancer. NKG2A/CD94-mediated inhibition of lymphocytes depends on the presence of specific leader peptides in MHC-E, but when and where they are presented in situ is unknown. We apply a nanobody specific for the Qdm/Qa-1b complex, the NKG2A/CD94 ligand in mouse, and find that presentation of Qdm peptide depends on every member of the endoplasmic reticulum-resident peptide loading complex. With a turnover rate of 30 min, the Qdm peptide reflects antigen processing capacity in real time. Remarkably, Qdm/Qa-1b complexes require inflammatory signals for sur-face expression in situ, despite the broad presence of Qa-1b molecules in homeostasis. Furthermore, we identify LILRB1 as a functional inhibition receptor for MHC-E in steady state. These data provide a molecular understanding of NKG2A blockade in immunotherapy and assign MHC-E as a convergent ligand for multiple immune checkpoints. Show less

Türünün ilk örneği olarak uzman yazarların okurlara siyasetten ekonomiye, toplumdan kültüre, cinsiyetten sanata kadar geniş bir konu yelpazesinde tarihsel kaynakları tanıttığı ve tartıştığı yüz yazı üzerinden Cumhuriyet Türkiye’sinin yüz yıllık serüvenini keşfetme imkânı sağlamak amacıyla yola çıktık. Amacımız, tarihin sayfalarını çevirirken, geçmişin anlarını ve anılarını yeniden canlandıran yasalar, mektuplar, gazete makaleleri, şiirler ve daha fazlası ile, her dönemin farklı atmosferini, geçmişin seslerini ve renklerini yakalamaktı. Show moreThe present study investigates the effectiveness of digital instructional games (DGBL) on alphabet signs (AS) and Word Learning (WL) in Persian to non-Persian-speaking children. The quasi-experimental method with a pretest-posttest design was applied to the study. Fifty-nine participants were randomly chosen from 400 students in Kalat-e Naderi, Iran. The participants were randomly divided into experimental and control groups. Descriptive and inferential (e.g., covariance) statistics were used to analyze the data. The findings revealed that DGBL significantly enhances AS and WL in Persian to non-Persian-speaking children. It is concluded that instructing AS and WL using the DGBL can improve the learners' performance in Persian learning. This conclusion may have pedagogical implications for language education programs, language instructors, and curriculum designers in academic settings. Show less

Show moreThis study examines how motivational signals in job advertisements relate to public employers’ recruitment success, testing hypotheses derived from signalling theory and person – environment fit theory. We combine job advertisements collected via web crawling with matched survey data on the recruitment process (1,863 recruiters), capturing Germany’s public sector job market. Using quantitative text analysis, we identify motivational signals in the job advertisements and analyse to what extent they relate to indicators of recruitment success. Results show that prosocial signals positively relate to a higher quality of applications and a more qualified selected candidate. By contrast, extrinsic and intrinsic motivational signals did not exhibit statistically significant effects. Show less

Show morePurpose: Predictors of 'imminent' risk of second hip fracture are unknown. The aims of the study were to explore strength of hip areal bone mineral density (aBMD), and muscle area and density for predicting second hip fracture at different time intervals.Methods: Data of the Chinese Second Hip Fracture Evaluation were analyzed, a longitudinal study to evaluate the risk of second hip fracture (of the contralateral hip) by using CT images obtained immediately after first hip fracture. Muscle cross-sectional area and density were measured of the gluteus maximus (G.MaxM) and gluteus medius and minimus (G.Med/MinM) and aBMD of the proximal femur at the contralateral unfractured side. Patients were followed up for a median time of 4.5 years. Separate Cox models were used to predict second hip fracture risk at different time intervals after first event adjusted for age, sex, BMI and diabetes. Results: The mean age of subjects with imminent (within 1st or 2nd year) second hip fracture was 79.80 +/- 5.16 and 81.56 +/- 3.64 years. In the 1st year after the first hip fracture, femoral neck (FN) aBMD predicted second hip fracture (HR 5.88; 95 % CI, 1.32-26.09). In the remaining years of follow-up after 2nd year, muscle density predicted second hip fracture (G.MaxM HR 2.13; 95 % CI, 1.25-3.65,G.Med/MinM HR 2.10; 95 % CI, 1.32-3.34).Conclusions: Our results show that femoral neck aBMD is an important predictor for second hip fracture within the first year and therefore suggest supports the importance concept of early and rapid-acting bone-active drugs to increase hip BMD. In addition, the importance of muscle density predicting second hip fracture after the second year suggest post hip fracture rehabilitation and exercise programs could also be important to reduce muscle fatty infiltration.
